Consistency & Connection

Over the course of a year, you’ll steadily build the healthy habits needed to help prevent type 2 diabetes. The program is divided into two key phases:

Phase 1: Months 1-6

We will focus on nutrition and physical activity fundamentals in our online group format. The first eight sessions are weekly, and then month 3 we move to bi-weekly. Access to a community forum helps us stay connected and communicating in between meetings! All meetings are virtual.

Phase 2: Months 7-12

We'll dive deeper into important behavioral topics like stress, sleep, and emotional eating. Based on your individual needs, you'll continue to receive support from your coach while working alongside your group to overcome challenges and encourage one another. These monthly meetings are virtual. Additional 1:1 appointments with your coach are offered as well.

Stage 1: Learning

The first step to building healthy habits is understanding where you are in your journey. We'll help you assess your routine and guide you through the essentials of nutrition, exercise, stress management, and sleep. With this foundation, you'll be able to identify the key areas to focus on for lasting change

Stage 2: Practice

Now that you have a clear understanding of your current habits, it's time to start making positive changes. Your Diabetes Prevention Program lifestyle coach will help you set specific goals, create a clear plan to achieve them, and provide motivation and support to overcome any obstacles along the way.

Stage 3: Advanced Learning

As you put your knowledge into action, we'll work with you to strengthen your routine. Whether it's a new exercise or a better way to manage stress, your healthy habits will soon become second nature as you build momentum in a healthy, active lifestyle.

Stage 4: Mastery

After identifying and tackling your barriers, you'll be ready for lasting reinforcement. In this final phase, you'll master long-term habit change and be fully equipped with the knowledge and confidence to overcome any obstacles that may come your way.
